Georgia taxpayers that need to request an installment agreement complete Form GA-9465. This form can be completed at the Georgia Department of Revenue website here. GA Form 9465 is no longer supported by the state to be completed within the tax software.
Typically, the Georgia DOR approves or denies the request within 30 days and if approved sends a notice to the taxpayer detailing the terms of agreement.
The form can be mailed, although the DOR recommends submitting it electronically at the Georgia Tax Center website here.
