Massachusetts has several important forms for charitable organizations registered to do business in the state. These forms aren't available in TaxSlayer Pro but can be gotten at the Massachusetts DOR website.

Massachusetts Form PC - This form must be filed annually by every organization registered as a public charity. Click here to complete it online. Massachusetts Form PC must be filed through the Massachusetts Charities Portal and cannot be filed in paper form. See here for more information.
Organizations should review the official Massachusetts Form PC instructions to confirm filing requirements and deadlines. - Form M-990T - Organizations filing federal Form 990-T may also need to file Massachusetts Form M-990T for unrelated business income tax reporting.. Additionally, an organization reporting unrelated business taxable income of $100,000 or more must file Form M-990T electronically in the current and all future years. This return is due April 15, and an extension can be requested for an additional 7 months.
- Form M-990T-62 - Instead of Form M-990T, this form is filed by exempt trusts and unincorporated associations with gross income from an unrelated trade or business subject to taxation under MGL ch. 62.
