Desktop: Forms not listed in the Form Finder

For tax years 2022 and prior, Form 1040-PR was the Spanish-language version of Form 1040-SS. (Beginning in 2023, Form 1040-PR is replaced with the Spanish version of Form 1040-SS.) The following instructions apply to tax years 2022 and prior.

To produce a Form 1040-PR, begin the return as a Form 1040 with the taxpayer's address in Puerto Rico. Then, from the Main Menu of the tax return (Form 1040) select:

• Personal Information
• Other Categories
• Bona Fide Resident of Puerto Rico - Answer YES.
• Print Return on Form 1040PR/1040SS - Choose 1040-PR.

To produce a Form 1040-SS, begin the return as a Form 1040 with the taxpayer's address in Puerto Rico, Guam, US Virgin Islands, or the Commonwealth of Northern Mariana Islands. Then, from the Main Menu of the tax return (Form 1040) select:

• Personal Information
• Other Categories
• Bona Fide Resident of [territory] - Answer YES.
• Print Return on Form 1040PR/1040SS - Choose 1040-SS.

Form 1040-SR, U.S. Tax Return for Seniors, is produced automatically if the primary taxpayer or spouse is over the age of 65. If it is included in the return, it will appear under View Results and will print with the rest of the return instead of Form 1040, however Form 1040 will also be available for printing under View Results.

Form 1040-X, Amended U.S. Individual Income Tax Return, is in the Form Finder under "AMENDED RETURN". It can also be gotten to from the Main Menu of the tax return selecting:

To access Form 2120, Multiple Support Declaration, from the Main Menu of the tax return (Form 1040) select:

• Personal Information
• Dependents & Qualifying Children
• Other button
• Form 2120 & Noncustodial Parent Dependent Selections

Form 2350 is used to request an extension of time to file. It is the equivalent of Form 4868 for a taxpayer who (a) is living abroad, and (b) expects to file Form 2555 and needs time to meet either of the two tests (bona fide resident or physical presence) to qualify for the foreign earned income exclusion and/or the foreign housing exclusion or deduction.

Irrespective of whether or not you have begun preparing the tax return, Form 2350 can only be created outside the tax return.

To indicate that you wish to file an extension request on Form 2350 rather than Form 4868, from the Quick Menu of TaxSlayer Pro select:

• The Extensions icon at the top of the screen
  or
• E-file
• Automatic Extension Application (Form 4868)

Enter the taxpayer SSN and follow the prompts, then in the Extension Personal Information window, select the button next to "2350 (Qualifying Taxpayers Abroad)".

Note that the Extension Personal Information window is the only place you can indicate the extension application is being filed on Form 2350 rather than Form 4868. If you bypass the last step above, you'll need to delete Form 4868 and start over.

Form 4852 serves as a substitute for either Form W-2 or Form 1099-R, and it is necessary when the payer hasn't issued one of those forms or issues an incorrect form.

To indicate that W-2 information entered in the return is obtained from a Form 4852, from the Main Menu of the tax return (Form 1040) select:

• Income
• Wages, Salaries, Tips (W-2)
• New - Select the check box next to h) Substitute and answer YES.

To indicate that 1099-R information entered in the return is obtained from a Form 4852, from the Main Menu of the tax return (Form 1040) select:

• Income
• IRA/Pension Distributions (1099R,RRB-1099-R)
• New - Select the Substitute check box on Line g and answer YES.

Form 5884-A, Credits for Affected Disaster Area Employers (For Employers Affected by Hurricane Harvey, Irma, or Maria or Certain California Wildfires) is used by taxpayers to claim the employee retention credit. This is a general business credit, and the current year amount, a carryforward, or a carryback can be entered by selecting from the Main Menu of the tax return (Form 1040):

• Credits
• General Business Credit (3800)
• Current Year General Business Credit, or
  General Business Credit Carryforwards, or
  General Business Credit Carrybacks
• Form 5884-A

Form 6198, At-Risk Limitations is produced in a return when two things are true:

• There is a loss on Schedule C, E, F, or K-1, and
• The investment in the business activity is indicated as not all at risk.

For a particular schedule where both of these conditions are true, when exiting the menu item for the business you will pass through the "Form 6198 - At-Risk Limitations Menu" where you can complete the appropriate lines on the form as needed. In addition, a Form 6198 menu line is available as needed for each property on Schedule E.

Form 6251 Exemption Worksheet for Line 5 is included in the Form 6251 instructions. It is produced as needed based on information provided elsewhere in the tax return and cannot otherwise be directly edited.

To view the entries being made on the worksheet, from the Main Menu of the tax return (Form 1040) select:

• Tax Computation (2021 and forward)
  Other Taxes (2020 and prior)
• Alternative Minimum Tax (6251)
• Exemption Amount

To access Form 8332, Release/Revocation of Release of Claim to Exemption for Child by Custodial Parent, from the Main Menu of the tax return (Form 1040) select:

• Personal Information
• Dependents & Qualifying Children
• Other button
• Form 8332

Form 8453 is required for an e-filed individual tax return when certain supporting documents are required by the IRS. The form will need to be printed and mailed to the IRS along with the required documents as specified on the form.

The form is generally produced automatically, based on information entered elsewhere in the return, with the check box next to the relevant form(s) checked, but there are four check boxes on the form that may need to be manually checked so that the form can be produced with the rest of the return.

To access Form 8453 for these four check boxes, from the Main Menu of the tax return (Form 1040) select:

• Mark Return Electronic
• Other E-File Information
• Mark Form 8453 Attachments
• Check the box as needed for:
  Form 3115, Application for Change in Accounting Method, or
  Form 5713, International Boycott Report, or
  Form 8858, Foreign Disregarded Entities Information Return, or
  Form 8283 Section A (if any statement or qualified appraisal is required)

Form 8582, Passive Activity Loss Limitations, is automatically produced if it's needed, as the entries on most of the form are pulled from information entered elsewhere in the return.

There are a few items that may need to be entered manually on the form. To access the form for these items, from the Main Menu of the tax return (Form 1040) select:

• Income
• Rents, Royalties, Entities
• Form 8582 - Passive Activity Loss Limitations - Select the line needing an entry:
• Gain reported on 4797 from a Rental Disposition - entered on Form 8582 Worksheet 1.
• Passive Limitation Calculation for Sch D Transactions - entered on Form 8582 Worksheet 3.
• Passive Limitation Calculation for Form 4797 Trans - entered on Form 8582 Worksheet 3.
• Passive Limitation Calculation for Form 6252 Trans - entered on Form 8582 Worksheet 3.
• Amount to Add/Subtract from Modified AGI - adjusts the amount on Form 8582 Line 7.

Form 8845, Indian Employment Credit, expired in 2021 and is only available in the General Business Credit as a carryforward.

To access Form 8845, from the Main Menu of the tax return (Form 1040) select:

• Credits
• General Business Credit (3800)
• General Business Credit Carryforwards
• Form 8845

Form 8878 is the signature form for e-filed extension requests. See here for instructions on how to create an extension.

To print Form 8878, from the Form 4868 Extension Menu select

• Send Extension Electronically? - change answer from NO to YES
• Form 4868 Electronic Payment
• View (icon in the toolbar at the top of the screen)

Form 8888 is used to divide the refund between a direct deposit and a check, to allocate the refund to two or three bank accounts, or to buy U.S. Series I Savings Bonds (in $50 increments). This form cannot be used in conjunction with a bank product. In TaxSlayer Pro, purchase of a Savings Bond requires at least a portion of the refund to be direct deposited.

In a tax return, Form 8888 is available once the taxpayer's direct deposit bank information has been entered, whether the return is being e-filed or paper filed.

To access Form 8888 in a return being e-filed, from the Main Menu of the tax return (Form 1040):

• Select Mark Return Electronic - If queried whether or not the taxpayer wants fees deducted from the refund, answer NO.
• Select Direct Deposit - Not a Bank Product
• Enter the bank routing number, account number, and account type.
• Complete the Practitioner Pin information as appropriate.
• From the E-File Menu, select Form 8888.

To access Form 8888 in a return being paper filed, from the Main Menu of the tax return:

• Select Payments, Estimates & EIC
• Select Direct Deposit Information
• Enter the bank routing number, account number, and account type.
• From the Direct Deposit Account Information Menu select Form 8888.

Enter the information in the form:

• Select Amount to be Deposited in First Account, and enter the amount. This amount will be deposited to the first bank account already entered.
• If a second direct deposit is desired, select Amount to be Deposited in Second Account, and enter the bank info along with the amount.
• If a third direct deposit is desired, select Amount to be Deposited in Third Account and enter the bank info along with the amount.
• If the taxpayer wishes to buy U.S. Savings Bonds, select the appropriate Savings Bond line and enter the information. Bonds are purchased in $50 increments.
• If any remainder is desired to be received in the form of a check, select Amount to be Refunded on a Check.

For tax years 2022 and prior, Form 8907, Nonconventional Source Fuel Credit, expired in 2013 and is only available in the General Business Credit as a carryforward up to tax year 2022.

To access Form 8907, from the Main Menu of the tax return (Form 1040) select:

• Credits
• General Business Credit (3800)
• General Business Credit Carryforwards
• Form 8907

For tax years 2022 and prior, Form 8909, Energy Efficient Appliance Credit, expired in 2013 and is only available in the General Business Credit as a carryforward up to tax year 2022.

To access Form 8909, from the Main Menu of the tax return (Form 1040) select:

• Credits
• General Business Credit (3800)
• General Business Credit Carryforwards
• Form 8909

Schedule A (Form 8936), Clean Vehicle Credit Amount, is attached to Form 8936 when a qualifying vehicle is added.

To access Schedule A (Form 8936), from the Main Menu of the tax return (Form 1040) select:

• Credits
• General Business Credit (3800)
• Current Year General Business Credits
• Form 8936, Part II or Form 8936, Part V
• Edit Qualified Vehicle Credit Information - Enter information about the vehicle.

Form 8959, Additional Medicare Tax, is produced as needed based on information provided elsewhere in the return. It cannot be directly edited.

To see the amount of additional Medicare tax calculated, from the Main Menu of the tax return (Form 1040) select:

• Other Taxes
• Other Taxes
• Additional Medicare Tax - 8959

The information in Form 9325, Acknowledgement and General Information for Taxpayers Who File Returns Electronically, is obtained from the acknowledgements received subsequent to a tax return or an extension request being e-filed. The form can be produced for a single taxpayer or for all taxpayer acknowledgements within a given date range.

To access Form 9325, from the Quick Menu of TaxSlayer Pro select:

• Reports
• Print Form 9325

To configure Form 9325 to automatically print with each acknowledgement received, from the Quick Menu of TaxSlayer Pro select:

• Configuration
• Electronic Filing / Bank Setup
• E-File Ack Setup Menu
• Print Form 9325 When Receiving Acknowledgments - answer YES

If a return includes Form W-7 and the preparer or another preparer in the same firm is a Certified Acceptance Agent, they can verify the applicant's documents without sending them to the IRS by completing and attaching Form W-7 (COA). The preparer's acceptance agent information must be indicated in their preparer record in the program configuration.

To complete this form, from the Main Menu of the tax return (Form 1040 or 1040-NR) select:

• Miscellaneous Forms
• Application for ITIN (W-7) - Select or create the application.
• Acceptance Agent / Certificate of Accuracy Information - Select, answer YES, select the agent, and enter the applicant's information as appropriate.

Form W-7 (COA) will print as a second page with Form W-7.

To print Form W-7 in Spanish, you'll need to indicate you wish to print return forms in Spanish instead of in English. To do this, from the Main Menu of the return select:

• Personal Information
• Other Categories
• Print English or Spanish Forms - Select Spanish.

If a return includes the Earned Income Credit, you can enter additional notes documenting your due diligence efforts. From the Main Menu of the tax return (Form 1040) select:

• Payments, Estimates & EIC
• Earned Income Credit
• Complete Schedule EIC
• Additional EIC Due Diligence Documentation

The Additional Due Diligence Documentation will print with the rest of the return.

When entering an asset in the Depreciation Module, you can indicate that you are opting out of bonus depreciation for the asset. This election statement will be print with the return and is also included in View Results for any assets so indicated.

The Qualified Dividends and Capital Gain Tax Worksheet is in the Form 1040 instructions and is produced if the tax return includes qualified dividends and/or capital gains.

The Clergy Worksheet refers to Worksheet 1 "Figuring the Percentage of Tax-Free Income" included in IRS Publication 517. It's used to determine a minister's taxable income as well as income subject to self-employment tax.

The worksheet can be accessed from either the Schedule C menu (if the minister needs to file Schedule C due to receiving ministerial income outside their wage) or the Schedule SE menu.

Schedule C

To make entries in the worksheet, from the Main Menu of the tax return (Form 1040) select:

• Income
• Business Income/Loss - Create a Schedule C or select an existing Schedule C
• From the Schedule C Edit Menu, select Answer Schedule C Questions
• Clergy Tax-Free Income Worksheet

Standard Deduction Worksheet for Dependents is in IRS Publication 501 and is produced if needed when the taxpayer, or the spouse if filing MFJ, can be claimed as a dependent by someone else. To indicate that the taxpayer or spouse can be claimed by someone else, from the Main Menu of the tax return (Form 1040) select:

• Personal Information
• Name & Address
• Dependent on Another Return - Answer YES.

This worksheet lists the depreciable assets included in Schedules C, E, or F. It prints with the schedule it is associated with.

This worksheet is required to calculate tax if the taxpayer claimed the foreign earned income exclusion, housing exclusion, or housing deduction on Form 2555 and is produced automatically when needed based on the calculations on Form 2555.

The EIC Worksheets A and B are available for printing in the View Results menu if the taxpayer qualifies for EIC. If the taxpayer doesn't qualify for EIC, the worksheets aren't available for printing in the View Results menu. The information in the worksheets that is used to calculate EIC, i.e., earned income and number of qualifying children, is pulled from where it is entered elsewhere in the return.

This due diligence worksheet is gleaned from information included in IRS Publication 501 and is produced for Head of Household filers if TaxSlayer Pro has been configured to produce it. In the return, it will be available to print under View Results, either by itself or in a print option.

To indicate that you would like this worksheet to always be produced in Head of Household filer returns, from the Quick Menu of TaxSlayer Pro select:

• Configuration
• Advanced Configuration
• Use Head of Household Worksheet? - answer YES

The questions on the Head of Household Worksheet are asked when the filing status of Head of Household is chosen in a return. Only as many questions are asked as are needed to determine whether or not the taxpayer qualifies.

The IRA Deduction Worksheet is included in the Form 1040 instructions and is produced if any amount has been entered here:

• Adjustments
• Traditional IRA Deduction
• Traditional IRA Contribution Made by Taxpayer

If a traditional IRA contribution has been entered, upon exiting the Traditional IRA Deduction menu the worksheet calculation is displayed on-screen. The bottom line of the calculation flows to Form 8606 Line 1.

Generally, a farming NOL must be carried back two years, with any remaining unused portion carried forward indefinitely until used up. (A nonfarm NOL can only be carried forward.) However, the taxpayer can elect to forego the carryback period and carry the NOL forward by filing an election with their tax return in the year of the NOL. This election is made under IRC Section 172(b)(3).

To make the election to forego the carryback period for a farm NOL in TaxSlayer Pro, from the Main Menu of the tax return select:

• Miscellaneous Forms
• Net Operating Loss Worksheet / Form 1045
• IRC Sec 172(b)(3) Election to Forego the Carryback Period - Answer Yes to the question.

The NOL carryback election will be sent to the IRS with the tax return and will also be printed with the return.

The Worksheet To Figure Your Qualified Loan Limit and Deductible Home Mortgage Interest For the Current Year is in IRS Publication 936 and is produced as needed based on information entered in the return.

This worksheet is included in IRS Publication 926 and is produced as needed based on information entered in the return. It isn't used if Schedule A includes a carryover of a prior year charitable contribution.

This worksheet was in the instructions for Schedule A (Form 1040) prior to 2018. It is obsolete for tax years after 2017.

The 28% Rate Gain Worksheet is in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.

The Capital Loss Carryover Worksheet is in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.

Publication 523, Selling Your Home, includes several worksheets useful for calculating (a) the basis of the taxpayer's main home that was sold, (b) the gain or loss on the sale, and (c) the maximum amount of the gain the taxpayer can exclude from income.

In TaxSlayer Pro, these worksheets are collectively referred to as the Sale of Main Home Worksheet, found under the "Other" button in the Schedule D menu. Click here for more information about entering the sale of the taxpayer's main home in TaxSlayer Pro.

The Unrecaptured Section 1250 Gain Worksheet is in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.

This worksheet is found in the instructions for Schedule D (Form 1040) and is produced as needed based on information entered in the return.

Taxpayers who are real estate professionals can elect to treat all their rental activities as a single activity rather than separate activities under IRC Section 469(c)(7)(A). To make this election in TaxSlayer Pro, from the Main Menu of the tax return (Form 1040) select:

• Income
• Schedule E
• IRC Section 469(c)(7)(A) Election - answer YES to the question. This will produce an election statement in the return, printable from View Results.

Note: This election continues beyond its initial year and can only be revoked in a year when the taxpayer's facts and circumstances have changed. This menu item is only used to make the election. Revoking the election is done by creating a preparer note to that effect in the return under Notes / Statements.

The Deduction Worksheet for Self-Employed is included in IRS Publication 560. To access the worksheet in TaxSlayer Pro, from the Main Menu of the tax return (Form 1040) select:

• Adjustments
• SEP, SIMPLE & Qualified Plans
• Taxpayer's SEP, SIMPLE & Qualified Plan Contribution
  or
• Spouse's SEP, SIMPLE & Qualified Plan Contribution

The Simplified Method Worksheet is included in the Form 1040 instructions and must be used to figure the taxable part of a pension or annuity for either of these two situations:

1. The annuity starting date was after July 1, 1986, and the taxpayer used the Simplified Method last year to figure the taxable portion;
2. The annuity starting date was after November 18, 1996, and both of the following are true:
• The payments are from a qualified employee plan, a qualified employee annuity, or a tax-sheltered annuity;
• On the annuity starting date, either the taxpayer was under age 75 or the number of years of guaranteed payments was fewer than 5.

The worksheet is needed if the SGR box is checked on a Form 1099-R. In the Form 1099-R input screen, the SGR check box is on Line 2a, Taxable Income, and after checking the box you will be asked if you would like to use the Simplified General Rule Worksheet. Answer YES, and enter the requested information as needed.

There are four Social Security benefit worksheets included in IRS Publication 915, and they will be produced in a tax return as needed based on the information entered in the return.

To enter Social Security benefits in TaxSlayer Pro, from the main menu of the tax return (Form 1040) select:

The Student Loan Interest Deduction Worksheet is included in the Form 1040 instructions and is produced as needed based on information entered in the return.
