Michigan Form 5680 (Fiduciary Income Tax Beneficiary Information Continuation Schedule) is used by estates or trusts filing a Michigan Fiduciary Income Tax Return (Form MI-1041) when there are more than four beneficiaries to report. It acts as an extension for Schedules 2, 3, and 4. Form 5680 is only needed if the beneficiaries received distributions.
To generate the Form 5680 in the TaxSlayer Pro program, from the main menu of the Michigan 1041 select:
- Taxable Income
- Fiduciary's share of Michigan net adjustments
- Was any income distributed to the beneficiaries - Change to Yes
