In your Account Hub at TaxSlayer Pro, the last step in the process of applying to use a bank product is verifying the information on IRS Form W-9. If anything needs changing, you'll need to send us a revised Form W-9.
You will also need to send us a revised Form W-9 if any of its information changes subsequent to being approved by the bank.
The easiest way to prepare Form W-9 to send to us is at the IRS website here. Once you have finished, you can either print the form to PDF and email it to firstname.lastname@example.org, or print the form to paper and fax it to us at 706.868.1955.
